Instructions for completing the form:

	Section 1- Account information
Contact Name	The Account name and the Contact name can be the same.
Phone (BTN)	BTN = "Bill-to Number"  This will become your account number at TouchTone.  If you sign up for another personal toll-free number, then be sure to use the same number again.
	Section 2 - To Request New
Destination Number (Ring-To)	This is the number that you would like to have your personal toll-free number forwarded to.
1st 2nd Choices	You can request a specific number for your personal toll-free number, but you may not get it. For example, 888-224-4323 spells "888-CallDad" on the keypad. Touchtone will try to get it for you, but most of the common ones are already gone.
	Section 3 - To Transfer
	Use this section if you already have a personal toll-free number and would like to transfer it to TouchTone because their rates are better.
	Section 4 - Special Instructions
Limited Access Block Access Extended Coverage	These are pretty much self-explanatory.
Prefer No 800	Important! You will pay for all incoming phone calls, including wrong numbers.  If you are requesting a phone for home use, then you should probably request one of the lesser known prefixes - 866-, 877-, or 888-. The phone company has trained customers to dial 1-800
